The most common withdrawal symptoms were anxiety, irritability or agitation, insomnia and psychosis, and 48.7% of the toxicity cases required intubation. US poison centers recorded 1,320 phenibut exposures from 2009 to 2019, with major effects in 12.6% of them and three deaths (Graves and colleagues, CDC's Morbidity and Mortality Weekly Report, 2020). Zvejniece and colleagues (Pharmacology Biochemistry and Behavior, 2015) then found in rat brain membranes that R-phenibut also binds the alpha2-delta subunit of voltage-dependent calcium channels, the site gabapentin binds. Its affinity there was about four times its affinity for GABA-B receptors, though far below gabapentin's (23 against 0.05 micromolar). In a rodent formalin pain test, a GABA-B antagonist did not block R-phenibut's anti-nociceptive effect, and the authors attributed it to the alpha2-delta site.
Lapin's review says phenibut was discovered and introduced into clinical practice in Russia in the 1960s. It describes the drug as widely used there for tension and anxiety, as a pre- or post-operative medication, and in conditions including asthenia, post-traumatic stress, stuttering and vestibular disorders. None of those uses has US approval.
